- Payment: Your information is used for billing and receiving payment from health plans and other entities. For instance, we use your information to bill your health insurance plan for services rendered.
We only share your personal information as required by law, contract, or as explained in this Privacy Policy. For example, we share certain data with patient health plans and Medflyt to facilitate our services, including information about patient visits, locations, time in/out, photographs, and signatures. We also share caregiver hours worked with Medflyt to facilitate billing and payroll.
Aggregated, anonymized data may be shared with external service providers to improve our Application and services. These providers work on our behalf and agree to adhere to this privacy statement.
We may also share your information when required by law for public health and safety, including instances where we believe it is required by law in specific jurisdictions outside the United States, provided it aligns with internationally recognized standards. Examples include reporting abuse, conducting research, complying with laws and regulations, and responding to legal requests.
We may use the information we collect to:
- Contact you with important information, required notices, and marketing promotions.
- Enhance your user experience, such as saving information for future visits.
- Improve, test, and monitor the effectiveness of our Application.
- Monitor metrics like visitor numbers, traffic, and demographics.
- Diagnose and resolve technology issues.
Regarding Biometric Information, we use it to identify caregivers and patients and verify time entries in the Application. We do not disclose this data unless you provide consent, it completes a financial transaction authorized by you or your representative, it’s required by law, or it’s pursuant to a valid warrant or subpoena. This information is retained for up to 10 years, after which it is permanently destroyed.
Dada Home Care / Family Wellness, LLC stores, transmits, and protects biometric data with reasonable care, equivalent to how we handle other sensitive information. If Dada Home Care / Family Wellness, LLC transfers its assets, your information may be among the items sold or transferred, and the buyer or transferee must adhere to this Privacy Policy.
Permitted Uses & Disclosures – Situational Disclosures
You have choices in how we use and share your information, such as updating your family and friends on your treatment, allowing us to market our services, and participating in fundraising efforts.
Sharing with Family/Friends: You can instruct us on how to share certain health information in specific situations, including with family, close friends, or others involved in your care or during disaster relief.
Marketing: We do not share your information for marketing purposes without your written consent.
Fundraising: While we may contact you for fundraising, you can opt-out of further contact.

- Request and receive a copy of your health record and the information we have about you.
- Correct or amend your medical record (subject to certain limitations).

- Designate someone to act on your behalf.
- Request limits on the information we share.

Duties of Dada Home Care / Family Wellness, LLC
We are obligated by law to safeguard your protected health information, promptly notifying you of any breaches. We will only use or share your information as described here, with your written consent. You can change your consent at any time in writing.
Your information collected through the Application may be stored and processed worldwide. By using the Application, you consent to this and the use and disclosure of your information as detailed in this Privacy Policy.
While we employ safeguards to secure information, we cannot guarantee its absolute security. User-provided data is retained for as long as you use the Application, with additional retention for up to 10 years for backup, archival, and audit purposes.
We may modify this Notice of Privacy Practices, notifying you before implementing changes. Your continued use of our services after any modification constitutes acceptance.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with Dada Home Care / Family Wellness, LLC or the Department of Health and Human Services (HHS) Office of Civil Rights. We will not retaliate against you for filing a complaint.
